Be part of a team where your skills and compassion help shape better health outcomes and strengthen community connections Responsibilities of this role will include: Promote cancer screening and early detection by supporting holistic screening days, community education, and embedding screening into everyday primary health care. Strengthen culturally safe referral pathways through collaboration with hospitals, AMS and palliative care services to ensure patients and families receive the right care at the right time. Engage with community by developing health resources, and delivering education to raise awareness and reduce barriers to care. Drive continuous improvement by collecting and analysing data, monitoring outcomes, and applying quality improvement processes to enhance service delivery. Support holistic, family-centred care by developing practical models of care, coordinating equipment and resources, and ensuring culturally appropriate palliative care is available close to home. To be eligible you must: Have full AHPRA registration with no restrictions Have a minimum of 12 months clinical nursing experience in either a practice or primary health care setting Current Drivers licence Experience working with Aboriginal and Indigenous communities - desirable Must have full working rights for Australia (i.e. Permanent Resident or Citizen of Australia/New Zealand, or a current, valid visa with no restrictions).
